What the inspector wrote

Licensing Program Analyst (LPA) Eldin Serrano conducted an unannounced visit to this facility to initiate a case management visit. LPA met with Licensee Lisa Tolentino and explained the purpose of the visit. The purpose of today's visit was to deliver an Immediate Exclusion letter for an individual named Ismael Loyola to the licensee. The exclusion of Ismael Loyola is based on a complaint of conduct inimical that has been substantiated. Based on evidence obtained during today’s visit the LPA has verified the individual is not present, employed or residing at the facility. LPA has advised the licensee to disassociate the individual from their roster and submit an updated LIC500 (staff roster). LPA reviewed the exclusion letter with the licensee explaining that Ismael Loyola is not allowed to be present in the facility. The licensee understands this Immediate Exclusion and has agreed Ismael Loyola cannot be allowed to work and/or live in a CCL Licensed facility and have contact with clients in any facility licensed by the California Department of Social Services. No deficiencies were cited during this visit. An exit interview was conducted with the licensee and a copy of this report and Immediate Exclusion letter was provided to the licensee Lisa Tolentino.
