What the inspector wrote

Licensing Program Analyst (LPA) Magda Malcore made an unannounced visit to the facility to conduct a required comprehensive annual inspection. LPA met with Administrator Heidi Vendiola, was granted entry, and discussed the purpose for the visit. The facility is a Residential Care Facility for Elderly (RCFE) with a license capacity of (6), and a current census of (3). LPA conducted a general inspection of facility, which included, but was not limited to, the following: Operation/Physical Plant: Indoor and outdoor passageways were kept free of obstruction. The facility has no swimming pools or similar bodies of water. The facility has sufficient indoor and outdoor space for resident activities. The facility is maintained at a comfortable temperature of 77 degrees F. Resident bedrooms were furnished with beds, night stands, chairs, bed linen and lighting. Resident bathrooms were maintained clean and fixtures were operating properly. The hot water temperatures in the bathrooms measured at 109 degrees F. The facility is equipped with operating smoke/carbon monoxide alarms, fully charged fire extinguishers, hallway nightlights, extra bed linen and towels. The facility has posted in a common area Community Care Licensing complaint poster, Ombudsman poster, facility license, resident personal rights, and emergency telephone numbers. LPA observed disinfectants and cleaning products were stored unlocked in facility hallway and under kitchen sink, accessible to residents in care. Food Service: Non-perishable and perishable food supply was sufficient for number of residents in care. Cups, plates, and utensils were sufficient for number of residents in care. Health Related Services: The facility maintains record of resident’s medications and medications were centrally stored in a locked cabinet. Record Review: Resident files review reveals resident#1 (R1) and resident#2 (R1) do not have preplacement appraisals on file for review. LPA observed R1 is utilizing bedrails. Administrator informed LPA that the bed with rails was provided by the hospital. LPA was not provided a physician's order for the use of half bed rails supports. R2 did not have record of tuberculosis results on file for review. Resident #3 (R3) last appraisal on file was conducted in 2017. Staff files were reviewed for First Aid/CPR certifications, criminal record clearances, training, and health screenings. Deficiencies were cited per Title 22 of the California Code of Regulations. An exit interview was conducted where this report and correction plans were discussed and a copy provided with appeal rights to Administrator Vendiola at the conclusion of the visit.
