What the inspector wrote

Licensing Program Analysts (LPAs) LaVette Farlow and Renese Howell-Small made an unannounced visit to the facility. The purpose of the visit was to conduct a required comprehensive annual inspection. LPAs met with caregiver Bernard Carpio and was granted entry to the facility. Caregiver Bernard notified Licensee of our arrival. Licensed capacity is (6) current census (3) with one of the three resident in the hospital and two (2) residents at the Day Program. The facility is vendorized by Inland Regional Center, Level 4I. LPAs were accompanied by Caregiver Bernard and Administrator Trupti Mody to conduct a general overall inspection, which included, but was not limited to, the following: Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCLD). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature. LPAs inspected resident bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, and sufficient lighting; bathrooms were clean, and appliances were operating appropriately. LPAs notice one (1) restroom did not have a slip resistant mat in the shower area. A technical advisory issued. LPAs observed sufficient furniture and lighting throughout the facility. The facility is equipped with operating smoke detectors and carbon monoxide alarms. Posters such as personal rights, the CCLD complaint poster were posted in a common area. LPAs observed the Licensee had the LIC 610E (Emergency Disaster Plan) but the Licensee did not review and update the report since 2019. A Technical violation issued. Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to residents in care. There was a designated space for residents/staff files. Overall, the facility is clean, in good repair, and operating in safe conditions for residents in care. ***Continued on LIC809C*** Food Service: Non-perishable and perishable food supply were low and not sufficient for number of residents in care. A deficiency was cited. Dishes, cups, and utensils were also stored properly. Care & Supervision: Facility has sufficient care staff for coverage 24 hours a day, 7 days a week. Record Review: LPAs reviewed (2) resident files for admission agreements, updated physician reports, and needs and services plans. LPAs audited 2 out of 2 resident P&I funds and observed the Licensee did not have the P&I funds available to audit. A deficiency cited. LPAs reviewed (2) resident medications. 1 out of 2 residents medication(s) and MARs did not match the record and medication storage. Licensee called the pharmacy during the visit to place the refill. A Deficiency cited. At the moment, the facility did not have Hospice residents. LPAs also reviewed (4) staff files for First Aid/CPR certification, criminal record clearance, training's, CPI and health screenings. The personnel file appeared to be maintained. LPAs review the facility first aid kit and LPAs observed the Licensee did not have scissors for the first aid kit. A technical advisory issued. Based on the observations made during today’s visit, three (3) deficiencies and (3) technical violation or technical advisory were cited per Title 22, Division 6, of the California Code of Regulations. An exit interview was conducted, and this report LIC809, LIC809C, LIC809D, LIC9102TV, and LIC9102TA were discussed and provided to Administrator, Trupti Mody.
