Licensing Program Analyst (LPA) Michelle Echeverrria conducted an unannounced case management visit to the facility to collect facility records. LPA was greeted and explained the reason for the visit to Administrator, Terronsay Whaley. This visit included an interview, collection and review of facility records and a walk through of the residents room. No deficiencies were observed during this visit. An exit interview was conducted where this report was, reviewed, discussed and then provided to Administrator, Terronsay Whaley.
