Third allegation states the facility does not have a working telephone on the premises. During the investigation and observations, the facility does have a working phone. During LPA’s visit the phone was ringing and staff was available to answer the phone. Fourth allegation states staff are interfering with resident visits. During the investigation LPA reviewed documents and interviewed (2) two staff members who stated the procedures for visitors. During the visit LPA observed several visitors coming and going and staff members were following their visiting protocol explained during the investigation. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED at this time. An exit interview was conducted where this report was discussed and provided to Morgan Williams at the conclusion of the visit.
