What the inspector wrote

Licensing Program Analyst (LPA) Becky Mann conducted an unannounced visit to the facility to initiate a complaint investigation. LPA Mann met with Michelle Reyes, Business Office Manager and explained the purpose of today's visit. The investigation consisted of LPA observations, pertinent document reviews, and interviews with staff. The allegation staff did not provide responsible party with resident’s records in a timely manner. LPA Mann interviewed Staff #1 (S1) and stated that at this time they are still unable to retrieve resident's records to provide to responsible party. Based on LPA observations, interviews and records review, the above allegation is Substantiated. A determination that the complaint is substantiated means that the allegation is/are valid because the preponderance of the evidence standard has been met. An exit interview was conducted where this report was discussed and provided to Michelle Reyes, Business Office Manager.
