What the inspector wrote

Licensing Program Analyst (LPA) E. Conchas made an unannounced visit to the facility to deliver two (2) complaint investigation deficiencies on complaint 56-AS-20251205105836. LPA met with Melissa Buckridge, Administrator and explained the reason for the visit. During today’s visit, LPA took a tour of the facility and requested copies of pertinent documents. Based on the previous complaints, (2) deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. An exit interview was conducted where this report was discussed, and a copy was provided to Melissa Buckridge, Administrator
