Interviews revealed that when a resident was identified as COVID-19 positive, the Resident Care Director would engage the isolation procedures and would report the COVID cases to the Executive Director. The Executive Director would then prepare a report to the corporate management company who was responsible for reporting any positive COVID-19 cases to the California Department of Public Health (CDPH). Interviews were not able to confirm if the facility’s corporate management team reported all of the COVID-19 cases to the CDPH. Interviews revealed that facility staff were not aware of any attempts by CDPH staff to contact them requesting updates for positive COVID-19 cases. The Department has investigated the above-mentioned allegation and based on interviews, the preponderance of the evidence has not been met, therefore, this allegation is deemed unsubstantiated. An exit interview was conducted with Executive Director Timothy Jeffers, to whom a copy of this report and the Licensee Appeal Rights (LIC9058 01/16) were provided via hard copy.
