What the inspector wrote

Licensing Program Analyst (LPA), Robert Campbell conducted a case management visit to the facility for the purpose of issuing deficiencies related to complaint investigation 18-AS-20260429153831. LPA Robert met with Alexa Cardenas - Caregiver and explained the purpose of the visit and assisted LPA. LPA toured the facility, observed, took pictures of the following physical environment Type B deficiencies, missing (plug plate Room#1) or broken plates for light switches in room numbers two (2), four (4), & five (5). LPA observed missing smoke detectors in the hallway between rooms three (3) to six (6) and a missing smoke detector in room five (5). Facility will review deficiencies on the 809D document. LPA during interviews of Administrator, two (2) Staff, cell phone video on Caregivers phones, that there is a violation of Resident#2 personal rights by videoing the Resident on cell phones in Resident#2 bedroom while having behaviors without consent. LPA will cite the facility with a Type B deficiency of violating the personal rights of Resident#2. Facility will review deficiency on the 809D document. An exit interview was conducted, and this report was discussed and provided to Alexa Cardenas - Caregiver.
