What the inspector wrote

On 08/18/2025, Licensing Program Analyst (LPA) Valerie Flores conducted a case management visit to issue a deficiency observed during the initial visit for complaint control number 18-AS-20250815131027. During this visit, LPA met with Administrator, Alyssa Legaspi. LPA conducted a record review for (4) four out of (4) four residents. Upon record review, (2) two out of (4) four residents maintained an ambulatory status of bedridden. Per the facility's fire clearance conducted on 11/2/23, the facility is permitted to retain a total of (6) six residents to which (4) four may be non-ambulatory and (0) zero bedridden residents. Administrator did not properly notify the fire department with the required 48 hours nor submit an application for an updated fire clearance. The facility will be cited in accordance with Title 22 Section 87202. An exit interview was conducted where a copy of this report was reviewed and provided to Administrator, Alyssa Legaspi, along with appeal rights.

This requirement was not met with evidence by: Based on interviews and record review, (2) two out of (4) four residents maintained a bedridden ambulatory status to which the facility can maintain (0) zero bedridden residents per the fire clearance. This poses an immediate health and safety risk to the residents in care.
