What the inspector wrote

On 7/15/2025, Licensing Program Analyst (LPA) Janette Romero conducted an announced visit to the pending facility to conduct a pre-licensing inspection for a change of ownership. LPA met with Administrators Daniel Mireles and Ailyn Joy Bulaoro who were informed of the purpose of the visit . On 5/12/2025, a fire clearance was granted for one (1) bedridden, two (2) ambulatory, three (3) non-ambulatory elderly residents. During today's visit, LPA observed five (5) residents and two (2) care staff present. LPA was informed four (4) residents are currently receiving hospice services at the facility. LPA toured the facility with the administrators. The facility is made up of a one-story home with five (5) resident bedrooms, two (2) resident bathrooms, a kitchen, living/family room and garage. No bodies of water were observed on the premises. Indoor and outdoor passageways are free of obstruction. An outdoor shaded seating area is available for the residents. LPA observed a charged fire extinguisher in the kitchen. Administrator Mireles tested one (1) of the smoke alarms/carbon monoxide detectors and LPA heard them to be operational. The facility also has a working telephone and staff present have a criminal record clearance. LPA observed locked areas for facility files, cleaning solutions, and knives. Resident bedrooms had the required bedding, furniture, closet storage, and lightin. Bathrooms had grab bars and non skid mats in the showers. Additional linens are available in a hallway closet. LPA toured the kitchen and observed facility had a two-day supply of perishable foods and seven-day supply of nonperishable food items. Medications are centrally stored in a hallway closet secured with a master lock. Living/family room has a working television and adequate seating in common areas. The facility has a central heating and air conditioning system installed with a central panel located in the hallway to control entire house. Emergency phone numbers/disaster plan, residents' personal rights, facility sketch, and complaint procedures were posted near the kitchen/dining room. The pending facility's certificate of liability insurance expires on 11/7/2025 and administrators have a valid administrator's certificate. During today’s visit, LPA did not observe any issues or concerns. Departmental records note Administrators Mireles and Bulaoro completed the COMP III requirement on 7/8/2025. Final approval of licensure will be granted by the Centralized Application Bureau analyst. An exit interview was conducted and a copy of this report was reviewed and provided to the administrators.
