What the inspector wrote

On 01/20/2026, Licensing Program Analyst (LPA) Janette Romero made an announced visit to the facility to conduct a pre-licensing inspection. A change of ownership application dated 12/01/2025 was submitted to the Centralized Applications Bureau (CAB) to operate a Residential Care Facility for the Elderly. LPA met with Applicant, Brenna Tomlinson whose administrator’s certificate expires on 09/22/2026. LPA reviewed a Fire Safety Inspection Request (STD. 850) dated 07/07/2025 noting the facility has been granted a fire clearance to accommodate six (6) non-ambulatory/ambulatory residents. During today's visit, there were five (5) residents and two (2) care staff present. Staff present have a criminal record clearance. LPA toured the facility's interior and exterior with Applicant Tomlinson. The facility is made up on a one-story home with five (5) resident bedrooms, three (3) bathrooms, a kitchen, dining room, living room, and attached garage. Indoor and outdoor passageways are free of obstruction. The facility has operating utilities and there are no bodies of water on the premises. Applicant tested one of the smoke alarms/carbon monoxide detectors and LPA heard it to be operational. The facility has a central heating and air conditioning system installed with a central panel located in the hallway to control entire house. All resident bedrooms are adequately furnished with a bed, chair, drawer/closet space and functional lighting. Bathrooms have non-skid mats and grab bars in the showers. The living and dining room have sufficient seating for the requested capacity. The living room fireplace is adequately screened to make it inaccessible for the residents in care. LPA toured the kitchen and observed the facility has a two-day supply of perishable food and seven-day supply of non-perishable food items. Knives and sharp instruments are secured in a locked kitchen cabinet. Medications are centrally stored in a locked cabinet in the administrative office. Emergency disaster plan, facility sketch, Long-Term Care Ombudsman's contact information, complaint phone number and residents’ personal rights are visibly posted near the front entrance and throughout the facility. During today's visit, LPA did not observe any issues or concerns, and no corrections are required. Applicant Tomlinson completed the COMP III requirement on 01/13/2026 at the Riverside Regional Office. Final approval of licensure will be granted by the CAB analyst. An exit interview was conducted, and a copy of this report was reviewed and provided to Applicant Tomlinson.
