What the inspector wrote

Additionally, a $500 civil penalty was assessed in an LIC 421BG. A plan of correction was jointly formulated with Executive Director, Vonda Boller. An exit interview was conducted with Executive Director, Vonda Boller, to whom a copy of this report, LIC 9099D, LIC 421BG and Licensee/Appeals Rights (LIC 9058) were provided.
