Licensing Program Analyst (LPA) Dawn Segura conducted an unannounced case management incident visit. LPA was granted entry into the facility and met with Kristen Kearnaghan, Executive Director, with whom she discussed the purpose of the visit. During today's visit, LPA and the Executive Director discussed three Unusual Incident Reports that were submitted to Community Care Licensing between August 11, 2022 and August 22, 2022. According to the reports submitted, the reported incidents of resident falls occurred between August 10, 2022 and August 17, 2022. No deficiencies were cited during today's visit. Copies of this report and Licensee/Appeal Rights were provided to the Executive Director at the conclusion of today's visit. Kristen Kearnaghan’s signature on this form acknowledges receipt of the rights and a copy of this report.
