What the inspector wrote

Licensing Program Analyst (LPA) Janet Ngallo conducted an unannounced health and safety visit in response to an incident report received by the department on 04/02/2026. LPA was greeted and allowed entry into the facility by Licensee Leonor Carmona. The incident report stated that due to water damage and a positive test for asbestos, the facility needed to be vacated for treatment. The treatment will begin on 04/06/2026 and end on 04/10/2026. The facility stated they were in the process of searching for temporary housing. LPA discussed the facility’s emergency evacuation and temporary relocation plan with the licensee. The licensee stated that the current temporary relocation addresses on file were incompatible and would not be used. LPA and the licensee reviewed the emergency evacuation plan together, and the licensee updated the plan during the visit and provided a revised copy to LPA for review. The licensee reported that two(2) residents will be relocated to Carpel Board and Care Facility located at 2073 Hanford Drive, San Diego, CA. The remaining four(4) residents will be relocated to a temporary rental home at 10983 Parkdale Ave, San Diego, CA 92126 from Sunday afternoon, April 5th, 2026 through Friday, April 10th, 2026. During the visit, LPA observed several fans positioned throughout the facility drying the floors. LPA conducted a tour of the interior and exterior areas of the facility and did not observe any health or safety concerns. No deficiencies were cited during the inspection. An exit interview was conducted with Licensee Leonor Carmona to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 03/22) were provided.
