Licensing Program Analyst (LPA) Janet Ngallo conducted a health and safety visit to the temporary facility location, 10983 Parkdale Ave, San Diego, CA 92126. The facility's permanent location is temporarily closed due to scheduled water damage treatment. Four residents were relocated to this residence and two were relocated to Carpel Board and Care Facility located at 2073 Hanford Drive, San Diego, CA. LPA was welcomed by, and identified themselves to caregiver Conni Lopez and discussed the purpose of the visit. LPA Ngallo conducted a tour of the facility and spoke with residents and staff. All medications for residents were locked and safely stored. No records were collected during the visit. LPA did not observe any health or safety concerns. No deficiencies were cited during the inspection. An exit interview was conducted with caregiver Conni Lopez to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 03/22) were provided.
