What the inspector wrote

Licensing Program Analyst (LPA) Dang Nguyen conducted an unannounced Required 1 - Year Visit. The facility file was reviewed prior to the visit. LPA was welcomed by and identified himself to receptionist Karen Eugene. LPA then met and discussed the purpose of the visit with Regional Vice President Julie Wiese. LPA conducted a brief tour of the facility and observed the staff and residents in care. In accordance with the Department’s Infection Control program, LPA provided technical assistance and observed and evaluated the facility's implementation of their COVID-19 Mitigation Plan, to include disinfection, screening protocols, and the use of personal protective equipment. No deficiencies were cited during this inspection. An exit interview was conducted with Wiese, to whom a copy of this report and the Licensee/Appeal Rights (LIC9058 01/16) were provided via E-mail.

  • Right to personal privacy in daily care

    Personal Rights of Residents in All Facilities: “Residents in all residential care facilities shall have all of the following personal rights: (1) To be accorded dignity in their personal relationships with staff, residents, and other persons.” Based on records and interviews, Licensee’s staff, through sexual harassment, violated the dignity of 1 resident [R1] of 96 in care, which posed an immediate personal rights risk to persons in care.

  • Right to freedom from abuse and neglect

    Additional Personal Rights of Residents in Privately Operated Facilities: “…residents in privately operated residential care facilities shall have all of the following personal rights:…to be free from mental, physical, or sexual abuse.” This requirement is not met, as evidenced by: Based on records and interviews, Licensee’s staff caused sexual abuse on 2 residents [R1 and R2] of 93 in care, which posed an immediate personal rights risk to persons in care.
