What the inspector wrote

Licensing Program Analyst (LPA) Debbie Correia conducted an unannounced case management visit at the facility. LPA gained access to the facility and identified herself to Receptionist LPA explained the purpose the purpose of the visit was to discuss an incident report which was received in our office on September 14th, 2022. The Incident report indicated on September 14, 2022 Resident#1 (R1) sustained an injury when R1 attempted to open a cabinet in their private bathroom. The cabinet was not properly secured and fell off the wall hitting R1's head. During today's visit LPA interviewed ED Lopez, secured records, and conducted a facility tour. Deficiency is being cited Per Title 22, Division 6, Chapter 8 of the California Code of Regulations and is listed on LIC 809-D. An exit interview was conducted with Executive Director Julia Lopez and a copy of this report, LIC 809D, LIC 811 and Licensee/Appeal Rights (LIC9058 01/16) were provided via email. An electronic receipt of confirmation was requested to be sent by the Licensee upon receipt of the documents.

Based on an interview with the Executive Director (ED) the facility did not maintain good repair to 1 out of 82 resident rooms. This posed a potential safety risk to residents in care.
