(a) Each licensee shall furnish to the licensing agency...and to the person responsible for the resident within seven days of the occurrence …any incident which threatens the welfare, safety, or health of any resident…This requirement was not met as evidence by: Based on observations, interviews and records review, licensee did not report two (2) incidents that occurred at the facility for two (2) residents (R1 and R2), which posed a potential health risk to 2 of 3 residents in care.
