What the inspector wrote

Licensing Program Analyst (LPA) Carmen Lopez conducted an unannounced case management visit to the facility and conducted a health and safety check for resident in care. LPA Lopez identified herself and was granted entry by Kate Sherlock, Receptionist. LPA stated the purpose of the visit and reviewed the basic elements of the visit with Kahina Hadibi, Assisted Living Director. The facility self-reported an Unusual Incident/Injury Report regarding resident #1 (R1) (See LIC 811 Confidential Names List) to Community Care Licensing on February 6, 2026, and the SOC 341 was submitted to the Department on February 5, 2026. According to the reports, a resident had reported to an outside agency that an unknown male staff person wanted to purchase them and also touched them inappropriately. During today’s visit, LPA spoke with staff and R1, and requested and obtained resident records. LPA informed Assisted Living Director Hadibi that possible follow-up visits or calls may be required for this incident. No deficiencies were cited during this visit. An exit interview was conducted with Kahina Hadibi, Assisted Living Director and a copy of this report, LIC 811 and Licensee/Appeal Rights (LIC 9058 3/22) were provided to the Assisted Living Director at the conclusion of the visit.
