Licensing Program Analyst (LPA), Jose De La Cruz conducted a Plan of Correction visit. LPA met with Licensee Pat Santilla and discussed the purpose of the visit. On 11/10/25, the licensee was issued deficiencies regarding care of a resident with diabetes that required glucose monitoring and insulin inyections. Today, LPA toured the facility and observed corrections have been made and deficiencies have been cleared. An exit interview was conducted and a copy of this report along with Licensee Rights (LIC 9058 03/22) were provided to Licensee Pat Santilla whose signature below confirms receipt of these rights.
