What the inspector wrote

Interviews conducted with staff and residents confirmed R1’s absence was due to being treated in a Skilled Nursing Facility (SNF). A review of facility records confirmed the location of R1’s room at the time of admission to the facility on July 16, 2020, had changed upon R1’s return from the SNF. Facility records also revealed R1 would be provided a 30-days written notice before substituting their room, unless R1 agreed to the request for a change, if it was required to fill a vacant bed, or the move was necessary due to an emergency. An Interview with the facility Administrator revealed there was no documentation of a room change addendum or a 30-day notice provided to R1. The Department has investigated the allegation of facility changed resident’s room accommodations without amending the admissions agreement. Based on evidence obtained, the allegation is substantiated. A substantiated finding means that the allegation is valid because the preponderance of the evidence standard has been met. A deficiency is cited in accordance of California Code of Regulations, Title 22, Division 6 Chapter 8, and is listed on the 9099D. An exit interview was conducted with Administrator, Rocio Granda and a copy of this report, LIC 9099D and Licensee/Appeals Rights (LIC 9058 01/16) was provided. However, interviews with facility staff could not confirm if medications had remained stored in R1’s room while being treated at a SNF. Records further reviewed, including R1’s list of personal inventory, did not include all of R1’s personal belongings that were alleged missing. Interviews with staff and residents were inconsistent and could not corroborate the allegation facility did not safeguard resident’s personal belongings. The Department has investigated the allegation of facility did not safeguard resident’s personal belongings. Based on evidence obtained, including interviews and records reviewed, the above allegation is determined to be unsubstantiated as the Department could not meet the preponderance of the evidence standard. An exit interview was conducted with Administrator, Rocio Granda and a copy of this report and Licensee/Appeals Rights (LIC 9058 01/16) was provided.
