.Licensing Program Analyst (LPA), Natasha Persaud conducted an unannounced Plan of Correction visit. LPA met with Administrator, Rocio Granda. On 04/18/24, the facility was issued a deficiency regarding basic services requirements. Some residents are unable to leave the facility unassisted but will attempt to elope when staff are not present in the front office to observe or after hours. The administrator purchased and installed an alarm system to assist when staff are not present in the front office and/or after hours. The alarm system is operable and has deterred residents from leaving unassisted. The deficiency has been corrected and cleared. No deficiencies were observed today. An exit interview was conducted and a copy of this report along with Licensee Rights (LIC 9058 03/22) were provided to Administrator, Rocio Granda whose signature below confirms receipt of these rights.
