What the inspector wrote

Licensing Program Analyst (LPA) Natasha Persaud conducted an unannounced Required Annual Inspection. LPA was greeted and allowed entry into the facility, and met with Staff, Felix Agpawa. The licensee submitted a request for a decrease in capacity from six (6) to four (4) elderly residents. The fire clearance was approved on 06/09/25 for two (2) non-ambulatory and two (2) ambulatory residents. Non-ambulatory may be in rooms #2, #3, and #4. Today, the facility was inspected and the appropriate non-ambulatory residents are in the approved rooms. An exit interview was conducted and a copy of this report along with Licensee Rights (LIC 9058 03/22) were provided to Staff, Felix Agpawa whose signature below confirms receipt of these rights.
