Licensing Program Analyst (LPA) Sabel Martinez conducted an unannounced Required Annual Continuation Inspection. The LPA introduced himself and disclosed the purpose of the visit to Caregiver Aster Bezabih. The facility was licensed for a capacity of six (6) non-ambulatory residents, of which one (1) may be bedridden in room # 1. The facility had a hospice waiver for two (2) residents. During the visit the LPA reviewed resident and staff records. A carbon monoxide detector, fire extinguisher, and telephone were present and operational. Per staff, fire rms, nor ammunition were kept at the facility. The facility was clean, sanitary and in good repair. Pathways were free of obstructions and slip hazards. Resident bedrooms contained the required furnishings. Due to time constraints, a return visit on a subsequent day is needed to complete the annual inspection. An exit interview was conducted with Administrator Todd Brooks, to whom a copy of this report, and Licensee/Applicant Rights (LIC 9058), were provided.
