What the inspector wrote

The Department has investigated the allegation that the Licensee failed to abide by the admission agreement and has found that, based upon interviews and record review, the preponderance of the evidence standard has been met. Therefore, this allegation is deemed substantiated. This deficiency is noted on the attached 9099-D, and is cited in accordance with the California Code of Regulations, Title 22. An exit interview was conducted with Ms. Nonay and a copy of this report, along with Licensee Rights (LIC 9058 01/16), were provided to the Administrator via electronic mail. An electronic receipt of confirmation was requested to be sent by the Administrator upon receipt of the documents.

This requirement was not met as evidenced by:Based on record review and interviews, the Licensee did not provide R1 and their representative with a 60 days' written notice to increase the monthly rate due to increased cost of living. This poses a potential personal rights risk to R1.
