Maintain required personnel records for staff

Based on observation and record review, the licensee did not comply with the section cited above in 3 out of 3 times as the on going training records were not in the employee personnel files which poses a potential health, safety or personal rights risk to persons in care.

Maintain records of centrally stored medication dosages

Based on observation, interview, and record review, the facility did not maintain a record of medication dosages for the Department to review. The licensee did not comply with the section cited above in [2] out of [2] persons which poses a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the facility staff CPR/1st Aid certification expired on 6/7/2023 and has not been renewed yet. The facility did not comply with the section cited above in [3] out of [3] persons which poses a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the facility did not have an updated record of drills conducted with staff that include the type of drill, date of drill and staff names who participated in the drill. The facility did not comply with the section cited above in [3] out of [3] persons which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, LPA was informed a resident-occupied masterbedroom has a bathroom that is frequently used by other residents. The facility did not comply with the section cited above in [1] out of [1] persons which poses personal rights risk to persons in care.

Care of Bedridden Residents: To accept or retain a bedridden person, a facility shall ensure the following: Each bedridden resident's record includes sufficient documentation to demonstrate that the facility is meeting the needs of the individual resident. This requirement was not met as evidence by: Based on conducted interviews and the review of the records, the licensee did not sufficiently demonstrate through documentation that R1’s rotation needs were being met by staff at all times, which posed a potential health risk to one out of four residents in care.

Admission Agreements: No licensee shall enter into any continuing care contract with any person without approval by the Department in accordance with Health and Safety Code, Chapter 10, Division 2. This requirement was not met as evidenced by: Based on records reviewed and interviews conducted, the licensee did not follow their agreement in meeting all of R1’s activities of daily living needs, which posed a potential health risk to one out of four residents in care.
