Inspector’s narrative

What the inspector wrote

On 11/06/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required inspections. LPA was greeted by caregiver and the administrator Arni Montazer arrived shortly after. LPA verified the facility contact information, and will update accordingly. The facility has an approved hospice waiver for (6) with (3) currently receiving services. The facility is also approved for (1) bedridden resident, with there being (1) bedridden resident residing in room # 2. There are two sheds on grounds that are being utilized for storage. The facility was observed to have the required postings and to be clean and clutter free. Staff present were observed to have fingerprint clearance and to be associated to the facility. The facility has a fully charged fire extinguisher that was last serviced on 07/18/25. There are no guns, ammunition or bodies of water on the premises. The medications, chemicals, and sharps were observed to be locked and inaccessible to residents in care. The hot water temperature was tested and found to be within regulatory limits measuring at 115.7 degrees Fahrenheit. LPA observed for there to be non slip mats inside the bathrooms. The facility is conducting emergency disaster drills on a quarterly basis with the last drill being conducted on 09/05/25. The resident bedrooms were observed to have the required furniture. The food supply was observed to meet with requirements of a 2 day supply of perishable and a 7 day supply of non perishable food items. LPA conducted a records review and observed for the facility annual fees have been paid and the governing body was observed to be in good standing. The resident files were observed to have admissions agreements and medical assessments. The staff were observed to have the required training such as valid CPR. The administrator Arni Montazer was observed to possess valid administrator's certification which expires on 08/12/26. Based on today's inspection there were no citations issued. An exit interview was conducted and a copy of this report was reviewed and provided to Arni Montazer, administrator.
