Licensing Program Analyst (LPA) Correia conducted an unannounced Case Management visit to perform a Health and Safety check on Residents in care. Upon arrival, LPA was greeted by front Receptionist Jasmin Essary, identified herself, and met with Executive Director (ED) Brandon Cho, to whom the purpose of the visit was explained. During the visit, LPA Correia, accompanied by ED Cho, conducted a facility tour, interviewed staff, and secured pertinent records. The facility tour revealed no immediate health or safety concerns. The facility was observed to be clean, sanitary, and in good repair. Resident 1 (R1) appeared in good condition, had visitors, and was attending a virtual call at the time of the visit. No deficiencies were cited during today’s visit. An exit interview was conducted with ED Cho at the conclusion of the visit. A copy of this report and the Licensee/Appeal Rights (LIC 9058, 3/22) were provided. The signature below confirms receipt of these documents.
