What the inspector wrote

Review of the Department clearance system revealed that all staff reviewed have a current background clearance. Review of staff files revealed that staff 1 (S1) did not meet the required annual training hours for 2020 or 2021. [Resident Care Director was provided with an LIC811 Confidential Names List to identify S1] Interviews revealed that S1 has been employed at the facility since 2020 and has not taken a leave of absence. The Department has investigated the above-mentioned allegation and based on record review, the preponderance of the evidence has been met, therefore, this allegation is deemed substantiated. The following deficiency is cited per CA Code of Regulations, Title 22, and noted on the attached LIC9099-D page. An exit interview was conducted with Resident Care Director Amy Salvador, to whom a copy of this report and the Licensee Appeal Rights (LIC9058 01/16) were provided via hard copy.
