What the inspector wrote

Licensing Program Analyst (LPA), Natasha Persaud conducted a Case Management -Other visit. LPA met with and discussed the purpose of the visit with Executive Director, Stefanie Ancheta. During today's visit, LPA briefly toured the facility and observed residents in care. On 05/01/23, Community Care Licensing received a self reported incident involving Resident #1 (R1). The incident occurred on 04/27/23, R1 was found on the floor in their room and sustained injury. The facility contacted 911 and R1 received medical treatment. R1 has returned to the facility. No deficiencies were observed today. An exit interview was conducted and a copy of this report along with Licensee Rights (LIC 9058 01/16) were provided to Executive Director, Stefanie Ancheta whose signature below confirms receipt of these rights.
