What the inspector wrote

On 07/31/25, Licensing Program Analyst (LPA), Javina George made an unannounced visit to the facility to conduct the required annual inspection. LPA met with Administrator, Kevin Calhoun initially, who then left the facilty. LPA conducled the inspection with caregiver Clariza Bawiin. At the time there was At the time of the visit there were 3 staff and 5 residents present. The facility has an approved hospice waiver for 4. There are currently 3 residents receiving hospice services. There is one (1) resident receiving home health services. Below are the observations made during today's inspection. The facility was observed to be clean, clutter and odor free. The food supply was adequate as there was a 2 day supply of perishable and a 7 day supply of non perishable food items. The smoke and carbon monoxide detectors were tested and were found to be operable. The facility was observed to have the required postings. A copy of the facility's liability insurance was obtained. The hot water temperature was tested and 106.1. The facility is conducting emergency disaster drills on a quarterly basis with the last drill being conducted on 4/14/25, a drill will be conducted, by the end of day today. There are no guns, ammunition or bodies of water on the premises. The medications, and chemicals were observed to be locked and inaccessible to residents in care.A records review all (5) resident and (3) staff files. The files were found to have the required documentation (admissions agreement, medical assessment and resident appraisal). Staff were observed to have obtained criminal record clearance, and Cardio Pulmonary Resuscitation (CPR) First Aid training. The administrator Kevin Calhoun possesses a valid administrator certification that expires on 1/26/26. No citations were issued during today's inspection. An exit interview was conducted and a copy of this report, was reviewed and provided to Clariza Bawiin, Caregiver .
