Lastly, a completed form LIC503 Health Screening Report signed by a Doctor of Medicine on 12/22/2025 established Administrator has no health condition that would create a hazard to the person, clients, children or other persons and Administrator is within normal limits. Based on interviews and records, there is not a preponderance of evidence to prove alleged violation occurred, therefore the allegation is unsubstantiated. An exit interview was conducted with Administrator Linda Cruz, to whom a copy of this report, and the Licensee/Appeal Rights were provided to via email.
