Licensing Program Analyst (LPA), Natasha Persaud conducted a Case Management- Incident visit. LPA met with Executive Director, Olga Loi, Administrator, Terri Bostian, and Director of Resident Services, Allison Crowers. During today's visit, LPA briefly toured the facility, obtained records, and interviewed staff. Facility reported Resident #1 (R1) eloped from the facility on 02/19/22. R1 was discovered at 8:30am walking outside the facility. The facility begins breakfast service at 7:30am. Staff assisted R1 to the dining room for breakfast. R1 did not sustain any injuries. No deficiencies were issued today. An exit interview was conducted with Administrator and a copy of this report, along with Licensee/Appeal Rights (LIC 9058 01/16), were provided to the Administrator via electronic mail. An electronic read receipt confirmation was requested to be sent by the Administrator upon receipt of the documents.
