What the inspector wrote

(Continuation of LIC9099) Based on the Department’s investigation of the above-mentioned allegations and the evidence obtained during staff interviews, and records reviewed, we have found that the complaint was unfounded, meaning that the allegation was false, could not have happened, and/or is without a reasonable basis. The allegations were not pertinent to this licensed facility and were out of the Department’s jurisdiction. The Department has cross-reported this complaint to the appropriate agencies for follow-up. The report was discussed, and an exit interview was conducted with Raquel Matthews, Director of Health and Wellness, and Meagan Milligan, Business Office Manager . A copy of this report along with Licensee/Appeal Rights (LIC9058 3/22) were provided to Director of Health and Wellness Matthews at the conclusion of the visit.
