[CONTINUED FROM LIC 9099] It was discovered that at the bottom of the landing/home page of the facility’s website, Licensee incorrectly listed “#372604267” as their facility number. [Their actual facility number is 374604267]. Although the error was of one digit, it was enough to impede the public’s ability to use “Search by Facility Number” on the Department’s website. Per California Code of Regulations, Title 22, Section 87206(a), Licensees “shall reveal each facility license number in all public advertisements, including Internet…” The intent of the regulation is for the public to have independent and unfettered access to view facility compliance history. Based on LPA observation, a preponderance of evidence exists, and the allegation is substantiated. A deficiency is cited per California Code of Regulations, Title 22; refer to the attached LIC 9099-D. A plan of correction was jointly developed. An exit interview was conducted with Kelly, to whom a copy of this report and the Licensee/Appeal Rights (LIC 9058 01/16) were provided via E-mail.
