What the inspector wrote

On 11/14/15 Licensing Program Analyst (LPA) Javina George made an unannounced visit to conduct a 1 year required visit. LPA met with Administrator Eloisa Karatas, and explained the purpose of the visit. The facility has an approved fire clearance for (6) ambulatory of which (3) may be non ambulatory and (1) bedridden. The facility currently has 3 residents that are receiving hospice services, there are no residents receiving home health services and there is (1) bedridden resident. The facility was observed to be clean and clutter free. The resident medications, chemicals and sharps were observed to be locked and inaccessible to residents in care. The emergency disaster drills are being conducted on a quarterly basis with the last drill being held on 08/06/25, and there is a drill due by the end of this month. The fire extinguishers were last serviced July 2025. The hot water was tested and measured to be 11.5 degrees Fahrenheit. The smoke and carbon monoxide detectors were tested and found to be operable. The resident bedrooms were observed to have the required furniture (bed, chest of drawers, lamp. There was extra linen and personal hygiene supplies. The food supply was adequate meeting the staple of 2 day supply of perishable and a 7 day supply of non perishable food items. A file review was conducted of staff and resident files. Staff files reviewed were observed to have criminal record clearance and to be associated to the facility. The staff have received the initial and ongoing training with valid CPR certification. The facility annual fees and the governing body was observed to be in good standing. The resident files had completed admissions agreements and proper assessments. Based on today's inspection there were no deficiencies observed. An exit interview was conducted where a copy of this report was reviewed and provided to Eloisa Karatas.
