What the inspector wrote

Licensing Program Analyst (LPA) Marisela Garcia-Centeno conducted an unannounced case management visit at the facility. LPA was greeted at the front entrance by Caregiver,Beatriz Valez and granted entry after identifying herself. LPA explained the purpose of the visit to Administrator, Karina Lopez. During the visit, LPA Garcia-Centeno discuss the Title 22 regulation requirement for background checks and association of individuals to the facility prior to first day of work. During the annual inspection a caregiver was associated to the facility. An exit interview was conducted with Administrator, Lopez a copy of this report, and Licensee/Appeals Rights (LIC 9058 01/16) was provided.
