Licensing Program Analyst (LPA), Kristina Ryan conducted an unannounced virtual visit to complete a complaint investigation regarding the above-mentioned allegations. LPA Ryan introduced herself, and explained the purpose of the call to Administrator, Sasha Hightower. On June 1, 2021, the facility had a change of ownership. The resident mentioned in the allegations resided at the previously licensed facility, which is now closed. The resident did not reside at the newly licensed facility, therefore; the determination of the complaint was unfounded, meaning that the allegation is false, could not have happened, and/or is without a reasonable basis. An exit interview was conducted. A copy of this report and Licensee's Rights (9058 01/16) were provided to the administrator, via electronic mail. An e-mail receipt confirms the receipt of these documents.
