What the inspector wrote

Licensing Program Analyst (LPA) Liliana conducted a case management visit in relation to a complaint investigation visit. LPA identified herself and was granted entry by the Administrator Regina Jackson- Patton, LPA discussed the purpose of the visit with the Administrator During today's complaint investigation visit, LPA conducted a records review on four (4) resident records. LPA observed that there were no Admission Agreements for two (2) of the residents and no pre-placement appraisals for (4) of the residents. Two deficiencies were cited on the attached LIC 9099-D page for failure to complete a proper pre-placement appraisal for Resident #1, #2, #3 and #4 and for failure to complete an Admission Agreement for Resident #1 and Resident #2. An exit interview was conducted with Regina and a copy of this report, along with Licensee/Appeal Rights (LIC 9058 01/16) were provided to Regina at the conclusion of the visit.
