What the inspector wrote

Residents interviewed revealed that staff assist them with showering. Resident Representatives interviewed did not disclosed concerns regarding showering. LPA was unable to conduct an interview with Resident 1 (R1). A review of records for the month of September 2022, October 2022 and November 2022 noted that showers were provided on R1’s assigned dates. It was alleged that facility does not provide activities for residents. An interview with Activity Director Magdalena Garcia revealed that activities are provided at the facility daily for residents. A review of the activity calendar for the month of October 2022 and November 2022, displayed daily activities Monday – Sunday, from 9:30 am to 3:30 pm. Per Activity Directory, participation is encouraged from all residents, however some of the residents do not like to participate. Interviews with additional facility staff indicated that activities are provided to the residents daily and that they really enjoy participating and engaging in the activities. Residents interviewed stated that they enjoy the activities at the facility. Some residents shared their favorite activities with LPA Williamson. During the visits of LPAs to the facility during the month of October 2022, residents have been heard and observed participating in activities with staff. On 10/20/22, while sitting in the Administrator’s office, LPA Patterson could hear the activity coordinator playing a movement game with the residents, encouraging their participation. On 10/24/22, LPA Williamson observed the residents finishing a balloon toss activity with Staff 1 (S1). LPA was advised that the activity assistant was absence and S1 filled in. On 11/03/22, LPA Williamson observed the residents in the lounge area participating in chair yoga with Activity Directors Magdalena Garcia and Kelly Ajdukovich. Interviews with the residents' representatives revealed no concerns regarding the activities at the facility. The Department has investigated the above-mentioned allegations and based upon evidence obtained during interviews and records reviewed, it is determined that there is insufficient evidence to prove that the allegations occurred. Therefore, these allegations are deemed unsubstantiated. An exit interview was conducted with Administrator Genoveva Guerrero, to whom a copy of this report and the Licensee's/Appeal Rights (LIC9058 01/16) were provided to Administrator.
