Inspector’s narrative

What the inspector wrote

Additionally, interviews with staff and residents revealed that Resident 1 (R1) was hospitalized in early January 2025, and R1’s responsible party was not made aware of R1’s hospitalization by facility staff and interviews revealed that R1’s responsible party had not had any contact with facility staff. Review of incident reports submitted to the Department revealed that the facility did not submit an incident report regarding R1’s hospitalization. The Department has investigated the above-mentioned allegation and based on interviews, the preponderance of the evidence has been met, therefore, this allegation is deemed substantiated. The following deficiency is cited per CA Code of Regulations Title 22 and noted on the attached LIC9099-D page. An exit interview was conducted with Caregiver Raymond Abedoza, whose signature below confirms receipt of a copy of this report and the Licensee Appeal Rights (LIC9058 03/22).
