Licensing Program Analyst (LPA) Sabel Martinez conducted an unannounced Case Management visit. The LPA introduced himself and discussed the purpose of the visit with Business Office Manager Sam Elizondo. Resident Care Director Elaine Nunes assisted the LPA, and Executive Director Emily Delbarre joined via telephone. During the visit, the LPA discussed eviction procedures and collected pertinent records. An exit interview was conducted with Elizondo, to whom a copy of this report, and Licensee/Appeals Rights (LIC 9058), were provided via email. An email mail receipt confirmed the documents were received by Elizondo.
