What the inspector wrote

Licensing Program Analyst (LPA) Amy Rodgers made an announced visit to the facility to conduct a case management visit to continue the annual inspection commenced on July 25, 2025. LPA identified herself, was granted entrance, and met with House Manager Patty Alonzo and consultant Henri Chazaud to whom she disclosed the purpose of the visit. According to the facility’s license, the facility has a maximum capacity of six (6) non-ambulatory residents, of which one (1) may be bedridden. This facility is divided into two dwellings which includes a separate additional dwelling unit (ADU) in the back of the property. During today’s visit, LPA, accompanied by Chazaud conducted a tour of the interior and exterior of the facility. LPA observed the facility was clean and in good repair. Pathways were free of obstruction and slip hazards. Required postings were observed. Resident bedrooms and bathrooms contained the required furnishings and safeguards. Water taps accessible to Residents in care measured with in regulation. Extra linens, hygiene supplies, and Personal Protective Equipment (PPE) were present.. Medications were housed in a locked cabinet. Cleaning supplies and other toxins were inaccessible to residents in care. There were no bodies of water on the facility property. The facility had ample space for activities, dining and visiting. Per house manager Alonzo, there are no firearms or other weapons on the facility premises. The facility had a 7-day supply of non-perishable and 2 days of perishable food. Based on today's continuation of the inspection, no deficiencies are being cited at this time. An exit interview was conducted, and this report was discussed with House Manger Alonzo and Consultant Chazaud who was advise a copy of the report and Licensee Appeal Rights will be provided after the conclusion of the visit. House Manger Alonzo signature on this form acknowledges receipt of reports.
