What the inspector wrote

Licensing Program Analyst (LPA) Marisela Garcia-Centeno conducted a case management visit to explain the process for closing the facility. During the visit, LPA spoke with Licensee/Administrator, Halat Barwari, via telephone. The Licensee, Barwari came to the facility and met with LPA. LPA Garcia-Centeno explained the procedures and process to close the facility. No violations were issued during the visit. An exit interview was conducted with Licensee Halat Barwari via telephone. A copy of this report and appeal rights (LIC 9058, dated 03/22) were left at the facility. The Licensee’s came to the facility and signed the report.
