What the inspector wrote

Licensing Program Analyst (LPA) Marisela Garcia-Centeno conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. During the visit, LPA spoke with Licensee/Administrator, Halat Barwari, via telephone and explained the purpose of the visit. According to the facility license, the facility is approved for a maximum capacity of six (6) elderly residents, age 60 and above, all of whom may be non-ambulatory. The facility also has a hospice waiver for up to four (4) residents. At the time of the inspection, there were no residents in care. Licensee Barwari informed LPA that although the facility was licensed over a year ago, she decided to return to school and did not pursue operating the RCFE as originally planned. The facility is not currently ready to accept residents, and required signage has been removed. The Licensee also stated she had considered renting the home and having another individual operate the RCFE under her license. LPA conducted a case management visit to discuss and explain applicable regulations and the process for closing the facility as the license for this facility is not transferable.. During the visit, LPA conducted a tour of the interior and exterior of the facility. A complete annual inspection was not conducted, as there are no residents in care and the RCFE will not be operating as planned. An exit interview was conducted with Licensee Halat Barwari via telephone. A copy of this report and appeal rights (LIC 9058, dated 03/22) were left at the facility. The Licensee, Barwari came to the facility and signed the report .
