What the inspector wrote

Licensing Program Analyst (LPA) Carmen Lopez conducted an unannounced complaint investigation and in conjunction conducted this case management visit due to additional inconsistencies found during the complaint investigation. LPA Lopez identified herself and was allowed entry by caregiver Myrta Mompremier . LPA discussed the purpose of the visit with caregiver Mompremier. During the visit on March 27, 2026, LPA observed that a bedridden resident, resident #1 (R1), was located in room #5 that was not cleared by the fire department for a bedridden resident. According to the facility sketch and the facility profile, the fire clearance only approved for bedroom #4 to be a bedridden room. According to the R1’s Physician’s Report (LIC602), resident is bedridden. Additionally, there were additional Title 22 regulations that LPA went over with the facility to include but not limited to: Reporting Requirements; Furniture, Fixtures, Equipment, and Supplies; Maintenance and Operation; and Personnel Requirements. LPA did provide the facility with TSP information as an additional source for assistance. Deficiency was observed and cited and technical advisories (TA) were provided during today’s visit and may be found on the LIC809-D and TA9102 pages of this report, along with civil penalties that were assessed. An exit interview was conducted, a copy of this report, CPs along with Licensee Rights (LIC 9058 03/22) were provided to Licensee Rose Dorvilus and caregiver Mompriemier at the conclusion of the visit.
