What the inspector wrote

Licensing Program Analyst (LPA) Marisela Garcia-Centeno conducted an unannounced visit to complete the required annual inspection. LPA was greeted by Administrator Abigail Ventura , and the purpose of the visit was discussed. All staff present have current criminal record clearances, and the administrator certificate is current. The facility is licensed to serve six (6) non-ambulatory residents , ages sixty (60) and above ; approved for one (1) bedridden resident and a hospice waiver for two (2) . The current census is two (2) non-ambulatory residents , one of whom is receiving hospice care. Both residents were observed to be well cared for, with no signs of neglect. During the visit, LPA, accompanied by Administrator Ventura, toured the interior and exterior areas of the facility. The facility was found to be clean, organized, in good repair, and free from clutter . Bathrooms and the kitchen were clean and free of odors. Hallways were clear of obstructions and slip hazards. No immediate safety risks were observed. Residents’ bedrooms contained the required furnishings. Doors, windows, screens, toilets, and showers were operational. Adequate supplies of linens and hygiene products were available. The facility provided sufficient space and equipment for dining, laundry, visitation, meetings, and resident activities. Food storage met requirements, with at least two (2) days of perishable and seven (7) days of non-perishable food available. (Continue at LIC809C) (Continue from LIC809) Cooking and dining equipment were present and in good condition. No sharp objects, toxic substances, poisons, or open-faced heaters were accessible to residents. Medications were properly labeled and stored. No bodies of water were present on the premises. Staff stated that no firearms or ammunition are kept at the facility. Smoke alarms, carbon monoxide detectors, emergency lighting, and the facility telephone were operational. The fire extinguisher was purchased on October 7, 2025 . First aid kits were complete and accessible. Required postings were displayed in visible areas. The room temperature was comfortable at 72°F . LPA interviews revealed no concerns. Resident and staff files contained the required documentation, and confidential records were secured in locked storage. Hot water temperatures at resident-accessible taps met regulatory requirements; the kitchen sink measured 104°F . The licensee provided proof of current and active business liability insurance. No deficiencies were cited during today’s visit. An exit interview was conducted with Administrator Ventura. A copy of this report and the Licensee/Appeal Rights (LIC 9058, 03/22) were provided during the visit.
