What the inspector wrote

Licensing Program Analyst (LPA) Arian Golbakhsh conducted a scheduled Pre-Licensing Inspection. The facility file and personnel report was reviewed prior to the visit. LPA was welcomed by, identified themselves to, and discussed the purpose of the visit to Licensee Kamran Banayan. Administrator Alice Millan arrived later during the visit. The facility's planned capacity is for six (6) non-ambulatory clients, one (1) of which may be bedridden. Bedridden resident may only reside in bedroom #3. As the facility is not yet licensed, during today’s inspection there were zero (0) clients in care. LPA and Licensee Banayan toured the interior and exterior of the facility and inspected each room. Facility sketch was consistent with the current layout of the facility. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. As this will be a change of location for the facility, resident furniture items will be transported during moving day once this facility location is licensed. Doors, windows, screens, toilets, and showers were in working order. Hot water temperature at taps accessible to clients were all compliant: kitchen sink was measured at 106.5F. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and client activities. The facility was prepared to contain at least two (2) days of perishable food, and at least seven (7) days non-perishable food. Utensils were present, however no cooking or dining equipment were present. At this time, the facility did not have a secured storage for knives and similar sharp objects, as well as for planned medication storage. [Continued on LIC 809-C] [Continued from LIC 809] No toxic chemicals or poisons were accessible to future clients. No pools or bodies of water exist on the premises. Per Licensee Banayan, no firearms or ammunition are kept at the facility. Carbon monoxide and smoke detectors were in working order. Fire extinguisher was purchased within the last 12 months. A first aid kit and emergency lighting were not present at this time. Required licensing postings were not observed in visible areas of the facility. While touring the backyard area of the home, LPA noted that the deck railing posed an immediate safety risk due to height concerns and safety for Dementia residents. As immediate health and/or safety concerns were observed during today's visit, LPA determined the facility is not yet ready for clients. LPA consulted with Licensee Banayan and offered additional preparation materials/resources. LPA will follow-up with Licensee Banayan to schedule a subsequent pre-licensing inspection visit. An exit interview was conducted with Licensee Banayan to whom a copy of this report and the Licensee/Appeal Rights (LIC 9058) were provided. Their signature below confirms receipt of these documents.
