Inspector’s narrative

What the inspector wrote

LPA Correia conducted a scheduled pre-licensing visit. LPA identified herself with Administrator (ADM) Natalia Rivera, Javier Garcia, and Vanessa Nunez, and explained the purpose of the visit. LPA, accompanied by ADM Rivera, conducted an internal and external tour of the facility. LPA observed a sufficient amount of non-perishable food, and the facility to be sanitary and in good repair. Resident room contained all the required furnishings; bathrooms were equipped with all safety requirements. LPA also observed a large outdoor shaded area for activities and visitors. Per Administrator Rivera there are weapons or ammunition or pools or accessible bodies of water to residents of care on the premises. LPA observations showed the facility had locked cabinets and drawers for sharp objects, toxic solutions, and medications. Component III was conducted during today's visit and the facility was granted fire clearance for a capacity of 6 non-ambulatory residents on 2/3/2026. Based on today’s visit there were no deficiencies observed. LPA recommends Licensure. LPA conducted an exit interview with the facility Administrators who were advised no deficiencies were observed and a copy of this report and appeal rights were provided at the conclusion of the visit.
